Who was Kuina?
Kuina is shown in the franchise as Zoro's childhood best friend who trained in the art of swordsmanship with him. Although this isn't mentioned at any point in the series, the anime explains that she is the daughter of Shimotsuki Koushirou, Zoro's master as well as hers.
Kuina and Zoro had made a promise when they were very young that they would train hard until one of them managed to become the best swordsman in the world. The girl was extremely skilled and had everything to achieve her goal with flying colors, but an accident along the way took her life before she could achieve her dream. How did Kuina from One Piece die?
In the fourth episode of the series, we learn a little more about Zoro's past and his motivations for becoming such a dedicated swordsman. On this occasion, his relationship with Kuina is explained, as well as the fact that the girl was killed in a tragic accident. But what accident was that? The series doesn't say how Kuina from One Piece died.
However, in the anime the answer is given to the fans. There, the girl's father tells Zoro that she had an accident on a ladder while looking for a stone to sharpen her sword so that she could continue training in order to keep her promise to Zoro to become the world's greatest swordsman.
Thus, the answer to the question of how Kuina from One Piece died was in a fall from a ladder. After her burial, Koushirou gives his daughter's sword to Zoro, indicating that he trusts him to carry on his daughter's legacy and not let her dreams die on the beach.
An interesting curiosity is that, years later, Zoro meets a naval officer called Tashigi who is extremely similar to Kuina. The resemblance between the two is such that it makes Zoro uncomfortable when he's in the same environment as her.
What is the history of One Piece?
Regardless of how Kuina from One Piece died, and whether she really died or just faked her death as unconfirmed theories suggest, her importance in Zoro's career is undeniable. Without the promise made between them, perhaps the boy would never have made it this far as a swordsman and been so important in the Straw Hat crew led by Mokey D. Luffy.
